Create a Service Request

Service requests are intended for members and are automatically entered into our case file system. Service requests are sent to the appropriate specialist for processing and we will do our best to respond to them at the earliest opportunity.

You can also send an informal enquiry regarding general issues and questions. In general, the response time for enquiries is shorter than for service requests.

We would also like to point out the possibility of booking a call with a specialist, if you think it would be more appropriate.

In order to handle service requests in a satisfactory manner, we ask for your name, social security number, email address and phone number. Information about submitted enquiries is saved in our database and case file system.
